Durant Medical Clinic was created by Tom Douglas, MD. Dr. Douglas received his medical school training in Galveston at UTMB. He received his Internal Medicine training with the University of Oklahoma Tulsa College of Medicine. Dr Douglas has more than 30 years experience in emergency medicine. He was grandfathered into Emergency Medicine Board Certification by ABEM. His experience includes emergency medicine, toxicology, internal medicine, occupational medicine, pain management, hormone replacement therapy with bioidentical hormones for both men and women. Dr Douglas treats all ages of patients from the youngest pediatric patient to the oldest geriatric patient.

BHRT

What is BHRT with pellets?

BHRT is a method of replacing hormones such as testosterone and estradiol with a hormone pellet that is produced at a licensed compound pharmacy. The pellets have molecules of testosterone or estradiol that are identical to the molecular structure of the same hormones that your body produces.

What are the steps involved in BHRT?

  • First, make an appointment to consult with the doctor.
  • Second, Lab work will be done and blood drawn to test for hormone levels.
  • Third, the doctor will discuss with you your lab findings and ask you for a day to do the pellet procedure.
  • Fourth, the pellet procedure is performed in the office.
  • Fifth, the patient returns for more lab work 6 weeks after the pellet procedure to ascertain what the new hormone level is.
  • Sixth, the patient is told when to return in the future (8-16 weeks) for the next session of hormone pellet insertion.

What is the procedure for inserting pellets?

Pellet insertion is a simple in-office procedure performed under local anesthesia. The fatty tissue under the skin, as well as the skin, is anesthetized with lidocaine and a very small tract is made through a small incision in the skin over the upper buttocks. Then, the pellets are inserted into the tract under the skin. The incision site is closed with steri-strips and a see-through bandage is placed over the incision site. The entire procedure is painless and takes about 15 minutes.

When can I have the BHRT procedure done?

You must first make an appointment with the clinic. The pellet procedure can be done anytime that the physician is available. This means that it is possible to make an appointment on a Saturday or after business hours Monday through Friday or during the business hours. The clinic is very flexible for these types of procedures.

BHRT for Women

Usually, the woman will receive a testosterone pellet and an estradiol pellet. She will also begin taking a progesterone pill orally each day. The physician will discuss the entire process with each woman.

BHRT for Men

Usually, the man will receive 5-10 testosterone pellets during the procedure based on his symptoms and his lab findings.

Referrals

We refer to the appropriate specialist when needed. We work with all specialists in the area. If you desire a referral, just ask and we will accommodate you.

Physical Exams

  • Annual Physicals, or check-ups include a complete physical exam plus screening labwork, and, if indicated, an EKG.
  • Work Physicals include a complete physical exam with emphasis on areas of the body that may get extra abuse on the job. For instance, if the employee is expected to perform lifting function (40 lb or more) on a regular basis, then a lumbar spine X-ray may be warranted. Work physicals also include screening labwork which may include a urine drug test (depending on the employer’s wishes).
  • School and sports physicals include a complete physical exam plus screening labwork.
  • Executive Physicals are special physicals that include a complete physical exam, labwork, EKG, chest X-ray, and other specialized labwork depending on each individual, plus a cardiac stress test. A very thorough history exam is part of this physical.
